1. In the "Target files" add "showimg.php" to the target files list.

2. In the "Save -> Naming" select "Use query string for file name" then check "Use value of this parameter for file name" box and enter "filename" in the corresponding box.

Here are the settings:

Target files: *.jp*
Exploration type: Regular -> Entire site
Excluded URLs:
/ad/
/avatar/

That's it. You can add more "Excluded URLs" to increase download speed by excluding other addresses that don't lead to images.
